Definition of Dialysis
Dialysis: The process of cleansing the blood by passing it through a special machine. Dialysis is necessary when the kidneys are not able to filter the blood. Dialysis allows patients with kidney failure a chance to live productive lives. There are two types of dialysis: hemodialysis and peritoneal dialysis.

Faculty Profile: Mary Leonard, MD, MSCE, conducts research on children and adults with kidney disease and outcomes related to bone health and nutrition
She recently completed a pilot study to assess the effects of whole body vibration in dialysis patients. She and others had successfully shown in animal models the beneficial effects of high-impact physical activity in which animals stood on a vibrating platform (low magnitude mechanical stimuli).

The proposed 6 month pilot and feasibility will evaluate LMMS as an anabolic stimulus to bone in 30 adults on hemodialysis. The intervention will consist of 20 minute daily sessions in the home standing on an active LMMS vibration platform or a placebo device that emits an audible hum suggestive of an active device. Each device contains an electronic monitor that documents adherence. The study will examine trabecular bone volume fraction (bone volume/total volume, BV/TV %) and architecture using microMRI, and cortical volumetric BMD and dimensions using QCT at baseline and 6 months. The hypothesis is that active LMMS will result in greater mean changes in trabecular and cortical parameters in hemodialysis patients. The proposed study will test the feasibility of conducting the intervention in dialysis patients and will generate preliminary data on rates of change in trabecular and cortical parameters.
Effects of vibration on peritoneal transport of solutes in continuous ambulatory peritoneal dialysis patients

We conclude that nitroprusside and vibration can increase the peritoneal transport of small molecular solutes, and vibration has less influence on the loss of protein in CAPD. It suggests that moderated movement may improve the removal of the small molecules in CAPD patients.
Limitation of small-solute exchange across the visceral peritoneum: effects of vibration.

OBJECTIVE: To evaluate the importance of the peritoneal membrane diffusion resistances to small solutes prevailing outside the capillaries in the visceral versus the parietal peritoneum during acute peritoneal dialysis (PD). CONCLUSION: In conclusion, the visceral peritoneal transport of small solutes is normally limited by the presence of diffusion resistance outside the capillaries, which may be markedly reduced by "stirring" of the dialysate by vibration. However, the visceral peritoneum apparently becomes increasingly important after vibration. The moderate increases in transperitoneal clearances of macromolecules occurring upon vibration, indicate that vibration may also increase the dialysate/peritoneal membrane contact and/or cause some recruitment of capillaries.
Tips on using Vibration Therapy with health issues:
1. Pace yourself with a moderate workout of 2-4 minutes at moderate speeds.
2. Try a few speeds 1-10...then if you are not experiencing any sharp pains, or shortness of breath, try speeds 15-20.
3. I would not attempt speeds any higher than 25 until you are confident that your body can handle it.
Note: You should know that there are two types of Vibration machines available; Linear/Lineal and Oscillating/Pivotal. They are both Vibration machines but the difference is how the vibration is 'delivered' to the body. T-Zone uses Oscillating Technology which is considered therapy technology used by Chiropractors, Physiotherapists, and other health care providers. It is not recommended that Linear/Lineal Vibration be used with people that have had major surgeries, or older and younger users. We always suggest that you get clearance from a Health Care Provider before trying any forms of exercise including WBV (whole body vibration).
